The Johns Hopkins Community Health Partnership
(J-CHIP) 

 Johns Hopkins Medicine (JHM) is committed to transforming its approach to health care delivery by launching and sustaining programs that will improve the quality and efficiency of healthcare delivered at Hopkins.  To meet this goal, JHM created the Johns Hopkins Community Health Partnership (or “J-CHiP”), on behalf of its East Baltimore campuses [Johns Hopkins Hospital (JHH) and Johns Hopkins Bayview Medical Center (JHBMC)] and their surrounding communities, and in response to the Center for Medicare and Medicaid Innovation (CMMI) Healthcare Innovation Challenge. JHM has been awarded $19.9M of support over 3 years by CMMI in support of J-CHiP to promote the deployment of care models that create new East Baltimore healthcare jobs and improve care quality and efficiency.
